MDS-UPDRS versus MMSE: comparing the use of these scales to evaluate cognitive impairment in patients with Parkinson's disease.
The MDS-UPDRS is the most widely used scale to assess cognitive impairment in Parkinson's disease (PD). The Mini-Mental State Examination (MMSE) is also frequently used for the same purpose. The goal of this study is to compare the MDS-UPDRS and MMSE in terms of validity, reliability, and clinical utility. At the University Hospital of Nantes, we evaluated 141 patients with PD, including 29 without cognitive impairment (CIND) and 112 with cognitive impairment (PDCI). At baseline and after 6-month follow-up, we administered a psychometric evaluation with the MDS-UPDRS and the MMSE. The psychometric properties were as follows: validity: 0.75 for the MDS-UPDRS and 0.76 for the MMSE; internal consistency: 0.90 for the MDS-UPDRS and 0.95 for the MMSE; interrater reliability: 0.93 for the MDS-UPDRS and 0.86 for the MMSE; reliability: 0.96 for the MDS-UPDRS and 0.99 for the MMSE; concurrent validity: 0.76 for the MDS-UPDRS and 0.61 for the MMSE; and discriminant validity: 0.73 for the MDS-UPDRS and 0.56 for the MMSE. On the basis of the standard deviation of the MMSE scores, the minimal clinically relevant difference was 10.4 points for the MDS-UPDRS and 8.8 points for the MMSE.
